JOB TITLE: CASE MANAGER
REPORTS TO: DIRECTOR OF SOCIAL SERVICES
LOCATION: BRONX, NY
The Case Manager assesses families needs, plans a course of action, links family to services and monitors progress for a 30 unit families with children homeless shelter. S/he must assure families receive appropriate information and referrals to address issues so that they can obtain and maintain housing.
DUTIES & RESPONSIBILITIES:
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
Bachelor Degree in Social Work or Human Services Field considered with 5 years of case management experience working with the homeless families with children some presenting with mentally illness, and/or substance abuse. Bilingual a plus. Strong relationship building skills required. Ability to work independently but know when to raise issues to supervisor. Excellent organizational and time management skills necessary. Ability to effectively multi-task and follow-up on a timely basis. Strong PC skills required including Word, Excel and CARES.
